New or Transfer I-20s

To submit a new application for I-20 the student must submit the following:

Copy of passport
Student's complete transcripts (translated into English with grading scale)
$900 money order made payable to "Department of Catholic Schools" OR $1025 money order if being mailed outside the U.S.
Proof of financial responsibility (foreign bank certified letter)
Foreign address and telephone number
Local address and telephone number (where student will reside)
Address of where the I-20 is to be mailed
Custodial adult form signed

To submit a transfer application please submit all of the above along with:

Copy of your current I-20 and I-901 receipt
$300 money order made payable to "Department of Catholic Schools" instead of the aforementioned fee OR, $425 money order if being mailed outside the U.S.

* Custodial Parent form is not needed if the student will be residing with a parent.
