New or Transfer I-20s

To submit a new application for I-20 the student must submit the following:
  • Copy of passport
  • Student's complete transcripts (translated into English with grading scale)
  • $900 money order made payable to "Department of Catholic Schools" OR $1025 money order if being mailed outside the U.S.
  • Proof of financial responsibility (foreign bank certified letter)
  • Foreign address and telephone number
  • Local address and telephone number (where student will reside)
  • Address of where the I-20 is to be mailed
  • Custodial adult form signed
To submit a transfer application please submit all of the above along with:
  • Copy of your current I-20 and I-901 receipt
  • $300 money order made payable to "Department of Catholic Schools" instead of the aforementioned fee OR, $425 money order if being mailed outside the U.S.
  • Download the SEVIS transfer form below to return to your current school (BAHS School Code is: LOS214F00670010
* Custodial Parent form is not needed if the student will be residing with a parent.
