Appryse Enrollment & Sports Physicals – 2025–26 School Year
Bishop Alemany partners with Appryse, a secure online system that manages student health records and athletic clearance. All student-athletes must have an active Appryse account and a current physical on file in order to participate in athletics.
Step 1: Enroll or Update Your Appryse Account

Returning families: Log in to your existing account and update your information.

Step 2: Complete & Upload Sports Physical

After completing enrollment, download the official Appryse physical form (with your student’s unique barcode).

Schedule your physical with your family doctor, or visit a local ExerCare facility for a walk-in sports physical.

Have your doctor complete the Appryse physical form. Forms must include the Appryse barcode.

Upload the form directly to Appryse or fax it using the number printed on the form.

Physical Expiration Policy
Once your enrollment is reviewed, you will receive an email confirmation from the Athletic Office. If additional information is needed, you will be notified by email.
